The scientifically supported effects of pomegranate
1. Blood pressure
A meta-analysis of eight randomised controlled trials of pomegranate juice found systolic blood pressure lower by 4.96 mmHg (95% CI −7.67 to −2.25, p<0.001) and diastolic lower by 2.01 mmHg (95% CI −3.71 to −0.31, p=0.021). [1] A larger 2023 meta-analysis of fourteen trials in 573 participants found a systolic reduction of 5.02 mmHg (95% CI −7.55 to −2.48, p<0.001). The effect was clearest at 300 ml a day or less — systolic −6.11 mmHg, diastolic −3.10 mmHg — and was not statistically significant above that dose. [2]
2. Potassium
Pomegranate juice carries around 213 mg of potassium per 100 ml. [3] Under EU rules a beverage is a source of potassium at 150 mg per 100 ml, and the authorised wording is that potassium contributes to the maintenance of normal blood pressure, to normal muscle function and to normal functioning of the nervous system.
3. Inflammatory and oxidative stress markers
A 2023 systematic review and meta-analysis of 33 randomised trials in adults found lower C-reactive protein (−0.50 mg/L, 95% CI −0.79 to −0.20, p=0.001), lower interleukin-6 (−1.24 ng/L, 95% CI −1.95 to −0.54, p=0.001) and lower TNF-α (−1.96 pg/mL, 95% CI −2.75 to −1.18, p<0.001), alongside lower malondialdehyde and higher total antioxidant capacity. [4]
4. Ellagitannins and the urolithin question
Punicalagins are barely absorbed in their original form. Gut bacteria convert them into urolithins over the following day or two, and people fall into three metabotypes: those who produce urolithin A, those who produce isourolithin A and urolithin B, and non-converters who produce neither. Which group a person belongs to shifts with age, diet and health status. [5] Commercial pomegranate juice carries roughly 1,370 mg per litre of ellagitannin-derived compounds, though the range between brands is very wide — measured punicalagin content across commercial juices spans 7 to 300 mg per litre [6], and total ellagitannins 151 to 2,374 mg per litre. [7]
5. Blood flow during and after exercise
In a randomised crossover trial in nineteen resistance-trained adults, a single 1,000 mg dose of pomegranate extract taken 30 minutes before exercise raised blood flow immediately after a repeated-sprint test by 18.49 mL/min, and increased vessel diameter after both bench press (+0.029 cm, p=0.025) and leg press (+0.042 cm, p=0.001). Repetitions to fatigue were not significantly different — the measured effect was vascular rather than a strength gain. [8]
6. Endurance in trained athletes
In a randomised crossover trial, 26 trained male cyclists took 225 mg of punicalagins a day for fifteen days. Total time to exhaustion increased against placebo (p<0.02), as did time to the second ventilatory threshold (p<0.001). Isokinetic strength recovery and inflammatory markers showed no difference. [9] This trial, like the one above, used a standardised extract rather than juice.
7. Memory in older adults
In the largest and longest trial of its kind, 261 adults aged 50 to 75 drank 236 ml of pomegranate juice or a matched placebo every day for twelve months. On a visual learning measure the placebo group declined significantly over the year while the juice group did not (F(2,257)=5.90, p=0.003, effect size 0.45). Verbal memory measures showed no difference between the groups. [10]
